Nurse Information

No students entering school, including those entering kindergarten, those from out-of-state and those from nonpublic schools, will be permitted to enroll (or attend) without proof of immunization, as determined by the Commissioner of Public Health. It is the responsibility of the parents or guardians to have their children immunized and to provide such proof to the principal.

Please note all student allergies on the registration form. In accordance with the TN Department of Education and the TN Department of Health guidelines, Bradley County Schools will adhere to procedures for Life-Threatening Allergies. A documented diagnosis is required for an individual student health plan. Bradley County Schools will not serve peanut

butter in the Child Nutrition Program. Children are not to trade or share snacks or food. If you would like to have a treat for birthdays this year, please contact the cafeteria manager to order ice cream or other baked treats.

Medication shall be administered only when the student’s health requires that it be given during school hours. All prescription medication must be brought to the school in the original container with the pharmacy label containing all the information needed. All non-prescription medications must be in their original containers with the child’s name affixed. A medication form must be completed in the nurse's office for written authorization to administer. Students are not allowed to transfer or carry medications of any kind other than an inhaler. Please contact the school nurse for more information.
